This is a full-time role for a Nurse Navigator in the Heart Failure Clinic at HCA Florida JFK Hospital. As a Nurse Navigator, you will play a key role in coordinating and providing patient care throughout the patient's heart failure journey. You will collaborate with a multidisciplinary team to ensure patients receive comprehensive care and education. This is an on-site position located in West Palm Beach, FL. Qualifications

• Registered Nurse (RN) license in the state of Florida
• Minimum of 2 years of nursing experience, preferably in cardiac or heart failure care
• Experience in patient education and case management
• Strong knowledge and understanding of heart failure and cardiac care
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a multidisciplinary team
• Strong organizational and time management skills
• Bachelor of Science in Nursing (BSN) preferred
• Heart Failure Certification (CHFN) or willingness to obtain within one year of hire
• BLS and ACLS certification
