HCA - Helping Celebrate Abilities is Hiring a Social Worker Near Johnson, NY
- Conduct social history evaluations as a member of HCA Diagnostic & Evaluation Treatment Team for children 3-5 years old.
- Complete ongoing progress reports and recommendations for professional social work services provided for birth-adult population.
- Attend community meetings or hold membership on community committees as deemed appropriate by supervisor.
- Provide Article 16 Clinic counseling
- Document all social work services.
- Complete and maintain paperwork and case records in a timely manner.
- Maintain working knowledge of OPWDD requirements.
- Other duties or programs such as Care Coordination as assigned.
- One of the following preferred:
- Licensed Master’s Social Worker Degree in NYS (preferred), or
- Licensed Clinical Social Worker Degree in NYS
- One of the following acceptable:
- Graduate student pursuing MSW in NYS, or
- BA in a related field
- Excellent verbal, written and active listening skills.
- Excellent organizational skills.
- Ability to resolve crisis situations in a positive manner.
- Knowledge of social work practices and social work ethics.
- Knowledge of the NYS Department of Health (DOH), NYS education Department and NYS Office of People with Developmental Disabilities (OPWDD).
- Knowledge of diagnostic and treatment services including occupational therapy, physical therapy, speech therapy, and special education.
- Knowledge of human behavior and performance; individual differences in ability, personality, and interests.
- Driver's license and reliable transportation preferred.
- Proficient with Internet Explorer; Microsoft Excel; Microsoft Outlook; Microsoft Word; and Windows Operating System.
While performing the duties of this job, the employee is:
- Frequently required to stand and walk.
- Regularly required to sit and use hands in a repetitive motion.
- Regularly required to talk and hear.
- Occasionally required to reach with hands and arms
- The employee must frequently lift and/or move up to 50 lbs.
