Summary: The Patient Scheduling and Care Coordinatorisresponsible for scheduling patient visits, assisting with coordination of client care and completion of specific tasks necessary for the smooth operation of clinical services in HCR's branch office located in Oneonta. The PSCC performs EMR-related tasks and generates reports.

The PSCC effectively participates in discharge planning regarding patient’s transitioning from/to acute care and home care settings. The PSCC also collaborates with hospitals to prevent patient rehospitalization. In collaboration with the PCP, they also are instrumental in managing patient referrals and coordinating home care services.

Minimum Requirements:

• Unexpired/unencumbered NYS LPN license required.
• Two years of LPN experience; preferably in a home care setting (LHCSA or CHHA), hospital or provider office.
• Demonstrates discretion in handling confidential (PHI).
• Proficient with staffing and scheduling.
• Proficiency in computer systems
• Comfortable with Microsoft Office suite of products (Office 365)
• Profiency with EMR systems; Home Care Home Base helpful.
• Excellent interpersonal, customer service, and communication skills.
• Excellent organizational skills.

Job Type: Full-time

Pay: $26.00 - $31.00 per hour
