Job Description Summary: The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Patient Care A.
Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness (es).
B. Regularly re-evaluates patient nursing needs.
C. Initiates the plan of care and makes necessary revisions as patient status and needs change.
D. Uses health assessment data to determine nursing diagnosis.
E. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
F. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
G. Counsels the patient and family in meeting nursing and related needs.
H. Provides health care instructions and education to the patient as appropriate per assessment and plan of care. Review Date: 01/31/2019
I. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
J. Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload.
K. Provides weekend and holiday coverage as assigned by the Case Manager/Clinical Manager.
Communication

• Promotes customer service with timely callbacks, listening and understanding, good communication and positive attitude.
• Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
• Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
• Communicates with multidisciplinary team to coordinate the care plan.

Additional Duties

• Participates in on-call duties as defined by the on-call policy. B. Ensures that arrangements for equipment and other necessary items and services are available.
• Instructs, supervises and evaluates home health aide and LPN care provided every two (2) weeks per state guidelines.
• Participates in orientation, in-services, and staff meetings as scheduled.
• Follow all privacy policies of Health at Home and maintain the confidentiality of protected healthcare information (PHI).

Position Qualifications

• Currently licensed in the agency state of operation as a Registered Nurse.
• Possesses and maintains current CPR certification.
• One (1) to three (3) years of nursing experience in a hospital setting or community health setting involving direct nursing care.
• Must have valid driver's license for the agency state of operation or surrounding area with appropriate auto insurance.
• Must be able to communicate articulately and comprehend written and verbal communications.
• Must be able to function efficiently in a stressful work environment.

Continuing Education Requirements: Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.
Environmental and Working Conditions Environmental Conditions: May be exposed to extremes of heat and cold in all weather conditions.
Working Conditions: May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances. May be exposed or occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. OSHA exposure category:
Category I Position includes tasks that involve exposure to blood, body fluids, tissues.
Required Personal Protective Equipment: as required by working conditions.
