Adherence to post discharge plan(s) of care and the holistic health and functioning of the patient/family. Serves as a member of the interdisciplinary health team. Collaborates with departmental/organizational support services. Assists in the development, maintenance and evaluation of the Prisma Health Medical Management Social Work Services and performance improvements.

Accountabilities
- Provides psychosocial assessment and interventions. Coordination and collaboration with ancillary services, providers, and agencies.
- Implementation/coordination of identified discharge planning/placement needs. Works closely with others in an effort to optimize quality and minimize cost.
- Provides post-discharge follow-up when deemed appropriate.
- Assists in development/maintenance and evaluation of clinical data. Interprets data and makes recommendations based on results.
- Utilization of paper and computerized charting.
- Supports product line/medical staff quality improvement initiatives.
- Rotational weekend and weeknight calls.
Minimum Requirements
- Master's Degree in Social Work
- 2 years - Experience in health-related area
- Licensed Master of Social Work (LMSW) or Licensed Independent Social Work (LISW). ACM preferred. BLS required. Current Heartsaver certification required.
- Psychosocial intervention skills. Analytical and problem solving abilities. Strong written, verbal, and telephone communication skills. Ability to work within interdisciplinary setting.
