JOB SUMMARY
The Health Federation of Philadelphia is seeking a skilled and knowledgeable individual to manage the Integrated Peer Models for Health Program , a program funded by the Pennsylvania Department of Human Services. This program supports capacity building of peer professionals serving the North Philadelphia Health Enterprise Zone (HEZ) to benefit populations facing multiple barriers to health and wellbeing, including chronic disease, mental health/substance use challenges, and Social Drivers of Health.
The Program Manager will supervise the Community Partnership Coordinator and work with the Program Director to manage a learning collaborative for Community Health Workers (CHWs), Peer Support Workers (CPS/CRS/CRFS), and their supervisors, provide support to partner health centers and Community Based Organizations (CBOs) utilizing the Connect4health (findhelp) referral platform, and support other program initiatives.
The ideal candidate will be proactive and organized, possess outstanding interpersonal and communication skills, and have experience in program coordination and training. Experience working as part of, supervising, or providing training to the peer workforce, is strongly preferred, as is experience working within CBOs, Federally Qualified Health Centers (FQHCs), or major health systems in the Greater Philadelphia Area.
JOB SPECIFICATIONS
Responsibilities/Duties
  • Provide weekly supervision and professional development support to the Community Partnership Coordinator
  • Work with the Project Director, partner organizations, learning collaborative advisory board members, and trainers to develop engaging and robust trainings and consultations for CHWs, Peer Support Workers, and their supervisors
  • Present components of learning collaborative trainings and/or lead learning collaborative meetings as needed
  • Support partner organizations in maximizing use of the Connect4health (findhelp) platform through partner meetings, user groups, communications, and troubleshooting
  • Oversee the Connect4health Community Engagement plan implementation process to increase Connect4health visibility and utilization in local community organizations across Philadelphia
  • Assist in data collection and reporting processes
  • Collaborate with the IPMH team on planning for sustainability and future growth
  • Represent the Health Federation at various stakeholder meetings.
Qualifications
Education
  • Bachelor's or Master's degree in public health, social work, nursing or related fields;
Skills/Experience
  • 3-5 years of experience in program coordination, implementation, and supervision;
  • Prior experience as a CHW/Peer or supervisor of peer workers, preferred;
  • Familiarity with North Philadelphia community organizations and healthcare providers in the area, a plus;
  • Excellent written and verbal communication skills;
  • Strong organization, time management, and partnership-building skills;
  • Outstanding interpersonal skills and a collaborative equity-centered mindset with a background in interacting with culturally and racially diverse people and organizations;
  • Demonstrated experience in utilizing basic evaluation strategies.
Work Environment: Standard office setting with extended periods at work station and periodic use of office equipment, with periodic meetings or community events.
Position Type and Work Schedule : Full time position, typical hours are Monday through Friday 8:30 am to 5:00 pm. This is a hybrid position - 60% in-person and 40% remote work.
Travel: Some local travel for monthly trainings and meetings may be required. Candidate must be able to effectively travel within Philadelphia, either via public transportation and/or in own vehicle.
Physical Demands: Able to sit and work at a computer keyboard for extended periods of time. Able to lift and move up to 30lbs periodically
