Position Summary: The Physical Therapist will be an integral member of our interdisciplinary team (IDT). This role involves coordinating and providing restorative and rehabilitative Physical Therapy services to participants both at our Center and in their homes. Working alongside the Primary Care Physician, rehabilitation team, and other IDT members, the Physical Therapist will focus on maximizing participant independence, and safety, and restoring or enhancing function.
Essential Job Duties:
- Assessment and Planning: Conduct Physical Therapy assessments in both facility and home settings to determine participants' Physical Therapy needs, preferences, and goals.
- IDT Participation: Actively engage in IDT meetings to develop and refine participant care plans.
- Intervention Delivery: Implement and document Physical Therapy interventions based on physician orders and/or agreed-upon care plans, including active and passive exercise, muscle re-education, gait training, functional training of activities of daily living (ADLs), and various therapeutic treatments (e.g., massage, heat, sound, water, light, or electricity).
- Supervision and Training: Oversee Therapy Assistants by providing necessary training, instruction, and guidance to effectively implement Functional Maintenance Programs (FMP).
- Education: Train and instruct staff, participants, and caregivers/families on the use and care of therapeutic equipment.
- Education: Bachelor's Degree in Physical Therapy required; Master's Degree in Physical Therapy or Doctorate in Physical Therapy preferred.
- Experience: Five years of Physical Therapy experience required; two years of experience with a frail or elderly population preferred.
- License: Valid California Physical Therapy License or proof of license eligibility required. Physical therapy degrees earned outside the US must be evaluated to the US equivalent.
- Certification: Current CPR certification preferred.
