Job Overview:
Enthusiastic Enrollment/Care Manager needed. The ideal candidate will play a crucial role with patient education and enrollment processes, ensuring a seamless experience for intake. · Conducts intake for the agency and maintains a small caseload, offering both in-person and remote services.
· Facilitates daily motivational orientations for newly referred patients, utilizing NY state guidelines.
· Maintains accurate documentation, case notes, and workflow through computer-based programs.
· Participates in meetings, staff development sessions, and in-service training.
· Conduct regular case conferences with outside providers on an as-need basis
· Meet members/patients in-person regularly, completing accurate and up to date documentation and consents for services
· Conduct monthly follow ups to address any issues, review ongoing needs, and to discuss any client issues and/or new insights
· Be able to work in and create a positive environment, manage providers, delegate and oversee tasks/duties, and address conflicts as they arise.
**Qualifications:**
- Associate’s Degree required
- Proficiency in Spanish required.
- At least 3 years of related work experience, preferably within a community-based organization or government sector serving under-resourced populations.
- Experience working with public assistance populations preferred.
- Strong platform skills to motivate and engage clients effectively.
- Excellent verbal and written communication skills, along with organizational and problem-solving abilities.
