JOB DESCRIPTION

Looking for IL residents who live in one of the counties listed below and are interested in doing field case management with the elderly and individuals with disabilities who are approved for in-home or nursing facility care.

1. Lake County

2. Livingston/McLean/Ford Counties

3. Randolph/St Clair/Monroe Counties

4. Northwest side Chicago

Hybrid Model:

Remote with the future possibility of onsite

• Through the use of clinical tools and information/data review, conducts comprehensive assessments of referred member's needs/eligibility and determines approach to case resolution and/or meeting needs by evaluating member's benefit plan and available internal and external programs/services.
• Application and/or interpretation of applicable criteria and guidelines, standardized case management plans, policies, procedures, and regulatory standards while assessing benefits and/or member's needs to ensure appropriate administration of benefits.
• Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.

QUALIFICATIONS

• Graduate from an accredited school of nursing.
• Active, unrestricted licensure as a Registered Nurse in a state.
• Current state's driver's license with the ability to travel to hospitals, skilled nursing facilities, patient's homes, and other sites where patients receive care as needed.
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.
• Candidates must be able to perform all essential job functions with or without reasonable accommodation.

Preferred Qualifications:

• Certification in Geriatric Nursing or Geriatric Case Management.
• 2 years of nursing experience, preferably with older patients.
• Proficient in computer use, the Internet and health information technology.
• Demonstrated excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem solving.
• Proven ability to work effectively both independently as well as a member of an interdisciplinary team.

Skills/Abilities:

Good interpersonal and organizational skills. Ability to work with detailed, confidential material, computer experience required. Ability to follow directions and work with precision under pressure. Maintains sensitivity, maturity, and flexibility in coping with difficult situations.
