Clinical Assessment Manager
HealthCare Support is actively seeking a Clinical Assessment Manager to fill an opening with a leading healthcare organization in the Bronx, NY.
Clinical Assessment Manager Duties

• Build relationships with referral partners to achieve enrollment goals, ensure high performance in pre-enrollment verifications, and educate stakeholders about PACE program benefits and CenterLight’s initiatives.
• Conduct pre-enrollment assessments and assist with intake activities, ensuring compliance with CMS, DOH, and PACE guidelines, and work with the quality review team to maintain accurate documentation.
• Oversee administrative functions, support staff development, and advise on process improvements, while regularly reviewing performance metrics and adapting to meet company goals.
• Monday – Friday 8:30 AM – 5:30 PM 

Required Qualifications for Clinical Assessment Manager

• BS in Nursing or other health-related area, preferred
• Active and unrestricted New York State RN license.
• 3 years of experience in geriatrics, home care, MLTC, MAP, D-SNP and/or PACE long-term care products
• Must have previous Uniform Assessment System of New York (UAS-NY) experience

Pay Details: $115,000 - $120,000 annually
