Healthcare Support is Hiring a Behavioral Health Care Manager Near Las Vegas, NV
Daily Responsibilities for Behavioral Health Care Manager:
- Coordination of member’s behavioral health care through the health care continuum
- Collaborate with member, family, provider, facility, and health plan care team (to include PCP, COC, Medical Directors, and Community Services) to develop and implement an ongoing plan of care to obtain best outcome
- Provide health education, coaching, and treatment decision support for members
- Meets with member in their home, at their providers’ office, or at inpatient facilities.
- Work location: Will need to go to members’ homes and the community in Clark County NV
- Schedule: Monday – Friday 8:00am – 5:00pm
- Contract: 3 Month Right to Hire
- At least one year Case Management is preferred.
- At least 3 years of experience in behavioral health setting or doing direct patient care is required.
- License required -LMFT, CPC, LCSW. Must have current, unrestricted license. CCM preferred. Must maintain licensure.
