Clinical Assessment Outreach Manager HealthCare Support is actively seeking aClinical Assessment Outreach Manager to join a thriving medical company.Daily Responsibilities for Clinical Assessment Outreach Manager:
Start of care visit and then the care team decides who it should be passed to provide service (ie NP, Community Health Nurse, Physical Therapist etcs)
Team will then create a care plan for the patient
Go to the office for state of care meetings once a month
Visits per day can vary but typically it will be longer visits - Asking questions (they can be 100 questions)
You will not be providing any care just doing the assessment
Active and unrestricted New York State RN license.
Experience in geriatrics, home care, MLTC, MAP, D-SNP and/or PACE long-term care products
Experience in Uniform Assessment System of New York (UAS-NY)
Knowledge of community-based long-term care services to dual eligible and frail adult population
Comprehensive understanding of federal and state regulatory requirements with respect to health care eligibility, coverage and services of various public programs
Experience in working independently without significant oversight and/or direction from a supervisor.

Pay Details: $115,000-$120,000 per year
