Healthcare Support is Hiring a Community Resource Coordinator - Field Near Little Rock, AR
Community Resource Coordinator IIHealthCare Support is actively seeking a Community Resource Coordinator II to fill an opening with a Managed Care Organization in Little Rock, AR. Daily Responsibilities for Community Resource Coordinator II:
Efforts will be tailored round key deliverables for the pilot
Opportunities for program development, leveraging best practices and direct feedback on program experience
Member/Provider Outreach (telephonic face to face)
Serving as the health plan liaison for new/existing community based organizations
Completion of health risk assessments/screenings
Addressing social barriers (i.e. closed loop referrals)
Shift: Mon- Fri between 8am- 6pm
Field position
Required Qualifications for Community Resource Coordinator II:
HS Diploma or GED
Clear and active Driver’s license, reliable transportation, and car insurance
Located in or very close to Pulaski County
Bilingual highly preferred
Community Health Worker Certification required
Working Knowledge of Social Determinants of Health (SDOH) barriers
Working Knowledge of geographic area(i.e. posted county/service area) and known Community Based Organizations
Working knowledge of motivational interviewing and care plan development/goal setting
General understanding of Microsoft office suite (i.e. work, outlook, excel)
Preferred Qualifications for Community Resource Coordinator II:
Bilingual highly preferred
