Field Pediatric RN Case Manager
HealthCare Support is actively seeking a Field Pediatric RN Case Manager to fill an opening with a Centene - Superior Health Plan in Amarillo, TX .
Daily Responsibilities for Field Pediatric RN Case Manager:

• Complete in-home assessment which includes:
• SAI Core A to Core Q
• ISP
• EDP
• Other forms as applicable: 1500 forms, SBP, EVV
• Assessing the member's current health status, resource utilization, past and present treatment plan and services, short- and long-term goals, treatment and provider options.
• Follow up on requests for items/services
• Speaking to providers

Required Qualifications for Field Pediatric RN Case Manager:

• At least 2 years of Pediatric experience required
• Case management or home health experience
• Ability to learn new technologies and enter assessments into program as they're being conducted
• Clear and active TX RN license
• Clear and active driving record, reliable transportation, and insurance

Preferred but not a must qualifications for Field RN Case Manager:

• Bilingual in Spanish
• Case management experience

Pay Details: $38.00 - $45.00 an hour
