Role Summary: As a Community Liaison, you will serve as a liaison between community partners such as but not limited to hospitals, physician practices, skilled nursing facilities, assisted living communities, as well as patients and their families. Your key responsibilities will include analyzing market trends, identifying potential partners, and fostering relationships. Your efforts will directly contribute to HealthFlex's market leadership and the delivery of essential, top-quality hospice services to the community.
