Onsite in Denver Office Required for all shifts

Full Time 8am- 8pm with rotating Sunday - Friday shifts

Job Summary and Qualifications

The Virtual Admission\Discharge Nurse demonstrates advanced knowledge, skills and understands the unique aspects of the population served in the care of his/her patients. Will utilize the nursing process to independently deliver nursing care to patients in order to achieve quality outcomes and communicate need for guidance/supervision based on self-assessment of strengths and limitations.

What you will do in this role:

  • Identifies and individualizes patient goals. Compares expected and achieved outcomes of nursing care utilizing the plan of care (POC) consistent with the patient’s status, individualizes patient plan of care based on admission and discharge needs.
  • Nurses will aid in the completion of admission health history and medication reconciliation in electronic medical record with the patient and/or designated family or medical power of attorney.
  • Nurse will provide short report of admission health profile to bedside nurse once admission is completed.
  • Nurses will interface with unit leaders, physicians, operations staff and others to ensure patients are ready for discharge.
  • Nurses will work to ensure a strong patient experience and provide patients with an opportunity to feel comfortable and educated when discharging from the facility.

Orientation Period: Minimum of 2 weeks contingent upon experience. Extensive orientation may be required and will be determined by Department Director/designee

What qualifications you will need:
  • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure.Current Basic Life Support (BLS).
  • Graduate of a certified School of Nursing. BSN strongly preferred.
  • Minimum of at least two years of RN experience in acute care setting.
  • Must have strong effective verbal and written communication skills in order to interact in a clear, concise manner with staff, physicians, and administration.
  • Typing skills sufficient to complete the job in a timely manner.
  • Must be skilled in time management in order to coordinate and prioritize multiple tasks.
