MINIMUM QUALIFICATIONS:

Education:

Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree or Doctor of Pharmacy degree

Experience:

Central Inpatient: currently functioning in a central inpatient capacity (comparable role) is required

Outpatient: currently functioning in an outpatient/retail capacity is required

Medication History: currently functioning in an outpatient/retail or medication history capacity is required;

Decentralized/Clinical:

  • Currently functioning in a decentralized capacity (comparable role), or

  • Completion of a Post-Doctorate Pharmacy Program (i.e., residency or fellowship; e.g., PGY1 or PGY2 American Society of Health System Pharmacists accredited pharmacy program), or

  • Board certification (e.g. Board Certified Pharmacotherapy Specialist)

Licensure/ Registration/ Certification:

Currently licensed to practice pharmacy in the state of Minnesota.

Knowledge, Skills and Abilities:

  • Ability to work independently with minimal direct supervision.

  • Ability to meet and maintain departmental productivity standards.

  • Ability to work cooperatively with health-system and pharmacy staff.

  • Ability to handle frequent interruptions and adapt to changes in workload and work schedule.

  • Ability to set priorities, make critical decisions, and respond quickly to emergency requests.

Decentralized/Clinical: Post-graduate training and/or board certification in area of specialty (e.g. Board Certified Critical Care Pharmacist, Board Certified Psychiatric Pharmacist, Board Certified Oncology Pharmacist, etc.)

Licensure/ Registration/ Certification:

Licensed preceptor with MN Board of Pharmacy is preferred for any pharmacist roles that will precept students and interns.

POSITION SUMMARY STATEMENT:

Pharmacists provide professional pharmaceutical services by: verifying orders, dispensing medications and related supplies, monitoring patient medication therapies, providing drug information to healthcare providers, overseeing the work of direct support personnel, and performing related duties as assigned.

Basic Patient Profile Review During Order Verification as Appropriate for Individual Medication Order (Not Overall Review of Profile) - Patient information: Age, Sex, Allergies, Dose, Weight, Height, Renal or hepatic function, Drug interactions, Duplications, Incompatibilities, Identification of auto-sub medications, and inappropriate orders requiring follow-up. Ensure non-formulary medication process is followed and correct strength/dosage form is dispensed.

In-Depth Daily Patient Profile Review - Review diagnosis, medical history, OP medications, previous admissions and corresponding progress notes; Duplicate therapy identification and correction; Appropriate dosing of medications; Review lab results; Antibiotic stewardship and culture analysis; IV to PO substitutions; Evaluate appropriateness of IV drips; OP medications are held/resumed based upon patient condition; Indication for anticoagulation & appropriate dosing; Evaluate need for DVT/GI prophylaxis; Adherence to guidelines (e.g., Hypertension, CHF, Diabetes, etc.); Follow-up on any unresolved issues.

PHYSICAL REQUIREMENTS:

Repetitive, extensive, and prolonged use of hands & fingers, viewing a computer screen and using a keyboard during assigned shift, viewing minute details while checking/verifying med orders, lifting and carrying (up to 40 lb.), stooping or kneeling, sitting, walking, or standing for long periods of time (4–8 hours) is often necessary. Must be competent to operate the equipment used in assigned areas.

ENVIRONMENTAL CONDITIONS:

Works in a pharmacy or in patient care areas where space is limited and crowded conditions make it noisy. Potential for exposure to hazardous and toxic substances. Potential for sticks or cuts by needles and other sharp items. Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used. Works independently during night shift. May be exposed to communicable diseases during patient contact. May encounter customers who are upset, angry or abusive.
