Job DescriptionHealthPartners is hiring for a full-time RN Lead II.
QUALIFICATIONS:REQUIRED:Graduate of an accredited school of nursing
Current RN licensure in Minnesota
Current RN licensure in Wisconsin within two months of hire
Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable
Strongly demonstrated written and oral communication skills
Ability to prepare scheduling based on clinic needs
Demonstrated computer skills necessary to perform lead duties
Demonstrated Skills Including:Ability to guide and monitor others in using the nursing process to provide care to patients and their families
Ability to document performance and provide effective feedback to employees
Ability to instruct coworkers on job duties
Ability to have difficult conversations with coworkers
Ability to motivate and influence staff to achieve department needs and goals
Strong organizational skills
Ability to establish and maintain effective working relationships necessary to perform lead duties
Ability to lead by example and answer questions
PREFERRED:One year full time employment as an RN
PHYSICAL REQUIREMENTS:Ability to sit or stand for prolonged periods at a time. Must possess speech, hearing and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently. Proficiency in other languages may be required. Some positions may have lifting requirements.

ACCOUNTABILITIES:Nursing Process:Definition
Nursing assessment is accomplished through interviewing, observation, and/or nursing procedures.
Problem identification defines the impact of health problems affecting individuals and families. Registered nurses distinguish between normal and abnormal findings in order to identify patient capabilities and limitations.
Planning is the process of making decisions and setting goals which are mutually acceptable to the nurse and all pertinent others.
Implementation is the process of providing comprehensive nursing care.
Evaluation includes assessment and reassessment of the patient’s progress toward goal achievement.
Nursing Care:Performance is up to standard when:
Demonstrates clinical competence in using the nursing process to provide care to patients and their families.

Definition:Care Coordination:Care Coordination is the ongoing support of patients and their families over time as they navigate care and relationships. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.
Performance Is Up To Standard When:Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans
Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care
Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention
Identifies and secures patient services needed for patient care
Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.
Education:Definition:
Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education.
Patient / Family / Community:Works in partnership with patient/family to identify patient and family needs and develop a plan for education and/or make appropriate nursing referrals.
Teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation.
Engages in ongoing evaluation of the effectiveness of teaching in terms of understanding and application.
Professional:Participates in patient care conferences, in-services and educational programs.
Keeps current on best practices related to nursing and patient care

Nursing leadership is the possession of skills and knowledge to manage situations, assess problems and facilitate solutions. It includes listening, decision-making, delegating, communication, motivating, facilitating, role modeling, critical thinking and problem solving.
Role Modeling:Serves as a role model in the enhancements of nursing practice and professional conduct.
Demonstrates flexibility in performing additional responsibilities to contribute to the improvements in patient care and the functioning of the care unit and other support services.
Provides support to professional and technical staff.
Goal Determination / Achievement:Formulates and attains personal and professional goals which promote individual growth, and support the organization vision, mission and values.
Decision-Making / Problem-Solving:Demonstrates ability to assess and problem solve.

Definition:Organizational Behaviors:Organizational behavior is the building and maintaining of solid interdependent relationships within the care unit, and throughout Group Health, Inc.

Teamwork:Identifies and works to resolve problems with other departments and individuals in a constructive manner.

Other tasks as assigned by supervisor.
LEAD DUTIES: – Leader will assign lead duties. Lead 2 must perform at least seven of the bolded items.
Hiring:
  • Recommends filling or not filling a vacancy or adding to staff.
  • Assists Supervisor in interviewing or in some instances conducts interviews without involvement of a Supervisor.
  • Recommends hiring candidate to Supervisor.
  • May communicate offer of employment from Supervisor to candidate.
Scheduling:
  • Prepares staffing schedule.
  • Employees call lead if absent or late.
  • Coverage for planned and unplanned absences.
  • After the semi-annual vacation bidding process, grants daily vacation requests.
  • Approves timecards completed by Employees.
  • May approve working of overtime if specifically delegated by Supervisor.
Evaluation:
  • Documents performance.
  • After the performance review has been discussed with and approved by the Supervisor, conducts the performance review with the Employee.
Performance Management:
  • Documents performance.
  • Communicates to Employee that performance is deficient or behavior is inappropriate.
  • Coaches Employee in improving and setting workable goals (does not include disciplining the Employee and could represent a step prior to an oral reprimand).
  • Attends discipline discussion with Supervisor and Employee to provide information relevant to the discipline, including attending grievance hearings.
Other:
  • Assists in preparing unit budget.
  • Assists a Supervisor/Manager in work unit annual planning.
  • Assigns work.
  • Trains Employees and sets up and schedules training procedures (not limited to leads).
  • Directs Employees.
Within the work unit, the Supervisor/Manager shall discuss with the lead(s) his/her appropriate lead job functions as outlined above and that a lead is not to be assigned job functions outside of those listed above.
In recognition of the responsibility of leads, they shall be given adequate time to perform these duties
