Practice Details
- There are 4 hospitals (Clare, Gladwin, Midland, Alma) and 8 counties (Clare, Gladwin, Isabella, Midland, Gratiot, parts of Montcalm, Bay and Saginaw) in our service area
- M-F 24-28 hours/week
- Collaboration with Hospice NP and clinical team
- Participate in weekly IDT meetings and other meetings as needed
- Admission reviews, DocuSign signatures, education, clinical consultation as needed
- Medical Director Oversight of Hospice Program
- Home and facility visits required at times
- Workspace available in Midland and Mt Pleasant, can also work from home
- Rotation of on-call after hours 1:3
- Serving the Mt. Pleasant community since 2000, Medical Center of the Future opened in 2017
- Timely access to adult and pediatric primary care
- Leaders in patient experience with Emergency Department and Ambulatory Service Center scoring in top decile
- More than 50 physicians and advanced practice providers in more than 20 different medical specialties
- Online scheduling for Lab, Mammography, Primary Care, Urgent Care, In-Person and Virtual Care
- Laboratory and Imaging services available 24/7
- 2022 | Medical Center recognized as Mt. Pleasant Chamber of Commerce Large Business of the Year
- 2021 | Opened second operating room in Ambulatory Surgery Center
- 2020 | MyMichigan Medical Center Mt. Pleasant celebrated 20th Anniversary
