HealthRIGHT 360 is Hiring a Substance Abuse Navigator - CENS Near El Monte, CA

Job Overview

Client Engagement and Navigation Services (CENS) will focus on increasing show rates for clients shared with state and County entities with whom they will be working. The CENS will achieve this aim by meaningfully engaging these clients, determining which SUD services are most appropriate for individuals based on an ASAM brief triage assessment for adults and an evidence-based screening tool for youth, motivating the individual to connect with the selected treatment provider, and ensuring that these individuals have the necessary support (e.g., transportation) and health coverage (e.g., Medi-Cal or My Health LA) to successfully connect with these treatment services.
The CENS will provide a critical liaison role in assisting vulnerable individuals involved in multiple systems to access necessary services to address their SUD treatment needs.

The CENS will provide high-touch (e.g., face-to-face) interaction to facilitate access to SUD treatment necessary for improving health outcomes, reducing frequent use of hospitals and emergency departments, obtaining employment, maintaining income benefits and housing, reuniting with children, and satisfying probation requirements or pre- or post-plea diversion from the Los Angeles Superior Courts. Additionally, the CENS will establish and maintain linkages to additional services to appropriately refer clients to service providers that address unmet needs that create barriers to successfully engaging in SUD treatment, such as poor health, domestic violence, and homelessness.

Key Responsibilities

  • Traveling to co- locations at various homeless shelters and permanent supportive housing programs in the San Gabriel Valley area.
  • Assist clients in entering data in the Department of Public Social Services YourBenefitsNow! – website for Medi-Cal enrollment or contacting a My Health LA provider to schedule an enrollment appointment.
  • Provide clients with a basic overview of SUD in order to increase the likelihood of a client’s understanding and follow-through with treatment.
  • Discuss the steps to take in case of relapse, including overdose prevention; provide HIV/AIDS education and referrals for HIV testing and treatment services; and discuss the availability of medication-assisted treatment for alcohol and opioid use disorders.
  • Conduct an ASAM triage assessment to determine if the adult client is in need of SUD treatment and the provisional level of care to which the client will be referred.
  • May also conduct additional assessments for ancillary service referrals if they have not already been conducted by the referring case worker, including, but not be limited to, vocational rehabilitation, education, transportation, housing, physical health, and other public social services.
  • Use the Youth and Parent screening tool to determine SUD risks for youth clients and refer them to the nearest appropriate youth outpatient SUD providers, based on client preference and need, for further assessment as appropriate.
  • Scheduling clients for a full ASAM assessment with a SUD treatment provider that matches the client’s needs and preferences using SAPC’s electronic Service and Bed Availability Tool (SBAT).
  • Responsible for entering data into SAPC’s electronic health record system so that client information is provided to the treatment provider before the client appears for the appointment, if possible.
  • Responsible for contacting clients to remind them of their upcoming provider appointments and will coordinate transportation to the SUD treatment provider, if needed.
  • Responsible for providing substance use disorder trainings to various community partners.
  • Responsible for conducting community outreach, and participation in job and health fairs to promote public awareness about substance use disorders and to inform consumers about SUD treatment options available through DMC-ODS.

About HealthRIGHT 360

Our Mission HealthRIGHT 360 gives hope, builds health, and changes lives for people in need. We do this by providing compassionate, integrated care that includes primary medical, mental health, substance use disorder treatment and re-entry services. History To address the thousands of adolescents and young adults that were streaming into San Francisco for the cultural revolution of the 1960s, Haight Ashbury Free Clinics (HAFC) opened its doors in 1967 as the first free medical clinic in the country. During the first week of operation over 400 patients were seen. HAFC has been an innovator in d...elivering primary health care services to many of the people who can least afford them. Health Care is a Right, Not a Privilege has been the guiding principle as well as its famous tagline. Walden House was founded in 1969 in the same Haight-Ashbury district of San Francisco to help homeless and runaway adolescents with substance use disorder problems. Today, Walden House treats people with mental health and substance use disorder problems at various residential and outpatient centers throughout California, including in-prison treatment programs, and facilities in San Francisco and Los Angeles, providing drug and alcohol treatment and mental health, vocational and housing services for people transitioning back into their communities. Like HAFC, Walden House has always served people who are uninsured, homeless and socio-economically disenfranchised, including those with HIV/AIDS. Haight Ashbury Free Clinics and Walden House have both grown over the years, becoming national models for community healthcare, substance use disorder treatment and mental health services. The organizations merged on July 1, 2011 to best serve the most vulnerable members of our community. On July 1, 2012, Haight Ashbury Free Clinics Walden House adopted a new name: HealthRIGHT 360. A Growing Family of Programs Asian American Recovery Services (AARS) joined the family of HealthRIGHT 360 programs in 2013. Founded in 1985, AARS has grown to serve thousands of people throughout San Francisco, San Mateo, and Santa Clara counties. In 2014, North County Serenity House of San Diego county and Womens Recovery Association (WRA) of San Mateo joined HealthRIGHT 360, continuing its leadership as a provider of gender responsive services for women and women with children. In 2015 Lyon-Martin Health Services became the third primary care clinic operated by HealthRIGHT 360, continuing its specialized care for women and transgender individuals. Tenderloin Health Services (formerly Glide Health Services clinic) became the fourth primary care clinic in 2015, providing care in San Franciscos Tenderloin neighborhood. Prototypes joined in 2016 expanding behavioral health care for women and children and services to survivors of domestic violence in Los Angeles, Orange and Ventura Counties. Strengthening services for women and girls in San Francisco, Womens Community Clinic joined our family of programs in 2017. Womens Community Clinic traces its roots to the Womens Need Center, a program of Haight Ashbury Free Clinics that closed in 1999 and reopened four months later with its new name. More
