We are in search of a PRN Social Worker to cover the Killeen, Georgetown and surrounding areas. The Social Worker will provide psychosocial support to our hospice's patient/family unit based on an assessment that identifies the needs, goals, interventions, and services indicated in accordance with the established plan of care and to utilize professional training and personal judgment in monitoring the psychosocial assessment process. Responsibilities:

• Responsible for the implementation of standards of care for medical social work services‪‪.
• Assess the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment.
• ‪‪Counsel patient and family/caregivers as needed regarding stress and other identified coping difficulties.
• Provides crisis intervention when necessary.‪‪
• Demonstrates teamwork and effective communication to accomplish patient, team, and Heart of Texas Hospice goals.
