Heartland Family Service is Hiring a Case Management Coordinator Near Omaha, NE
Intensive case management and case coordination services will be provided to pregnant and postpartum women with children up to age 10 with the purpose of coordinating and attaining supportive family-based services, substance use and mental health care and physical health care goals. Decision making, problem solving, and highly developed interpersonal skills are critical.
Compensation: between $19.74 and $22.71 per hour (wage is determined by total years of relevant experience)
Work Schedule: Monday through Thursday, 8:30 a.m. to 5:00 p.m., Friday 8:30 a.m. to 2:30 p.m., may require some some evenings and weekends
