**SUMMARY DESCRIPTION OF WORK**

This position is a fully integrated Nebraska Family Works (NFW) team member who provides highly individualized services in the community and promotes client self-determination and decision-making from a whole health perspective. They provide supportive and case management services to persons with mental health and substance use needs with the purpose of coordinating and attaining health goals. The position begins working with clients shortly before discharge from Family Works, our residential substance use treatment program for women and children. Recovery Support Specialists also provide essential expertise and consultation to the entire team to promote a culture in which each client's point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help activities. Professionals at this level have excellent communication skills with clients, family members and community institutions. Decision-making, problem solving, and highly developed interpersonal skills are critical. This is a full-time position.

**Work Schedule: 3**7.5 hours, Monday through Sunday, day/evening hours, individual can help create their work schedule. **Compensation:** $17.95 to $23.33 per hour (based on experience)

**ESSENTIAL DUTIES AND RESPONSIBILITIES**

* *Works directly with member to assess holistic health care and social service needs, member strengths, abilities and desires.

* *Develops care coordination plan with the member that is realistic, achievable, goal oriented, and focused on member needs and desires

* *Interacts with primary care providers, behavioral health providers and other social service providers to make needed connections for services, provide a conduit for information sharing and organizes case conferences to bring providers together to review the care of mutual members.

* *Works within the team model, utilizing the expertise of the team members when appropriate for each member.

* *Provides problem solving and crisis intervention services to empower clients to develop skills necessary for a healthy living.

* *Coordinates with housing providers and assists with transition, move and/or acquiring household items upon discharge from residential program.

* *Consults with community agencies and organizations, including peer run programs such as self-help groups and 12-step groups. Facilitates client participation in peer self-help programs and activities for behavioral health and independent community living needs.

* *Documents client progress to maintain a permanent record of client activity according to the agency's established methods and procedures. Participates in daily staff organizational meetings and treatment planning review meetings.

* *Implements or develop a crisis relapse prevention plan with all clients.

* *Works in the community, in member homes, attends meetings and assists members as needed.

* *Excellent verbal and written skills in working with members and providers including primary care physicians.

* *High energy, passion for serving people, wants to make a difference, assertive, willing to push normal processes to enhance care of members

* *Excellent skills with a computer to document information and data.

* Demonstrates good interpersonal skills in developing rapport with clients via telephone and in person.

* Demonstrates knowledge of current community programs and resources and current community human service resources.

* Assist clients to plan and carry out socialization and leisure time activities.

* *Organizes and leads individual and group social and recreational activities ie sober support group, mentoring.

* Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.

* Is dependable and punctual regarding scheduling and attendance.

* Abides by all specific program and Agency procedures, policies, and requirements.

* Develops personal and program related skills through participation in internal and external training opportunities.

* Creates, maintains and shares an appropriate a dynamic self-care plan.

* Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agencys ongoing fundraising and friend-raising efforts.

* Essential functions of this job is to be performed on company physical work site.

* Performs other program related duties as assigned.

**MINIMUM QUALIFICATIONS**

* Bachelors Degree in social work or related field, or two years of direct care experience in the human service field.

* Two years lived experience with behavioral health. Paid or volunteer experience working with individuals with mental health and substance use preferred.

* Peer Support certification preferred.

* Valid drivers license/ acceptable driving record.
