Title: Clinical Case Manager

Location: Hybrid (4822 N Broadway and 839 W 79th Street)

Shift: Monday to Friday (8:30am- 5:00pm)

Summary:

We're hiring a Clinical Case Manager to provide specialized case management services within our organization. This role operates independently, ensuring adherence to professional standards, organizational policies, and contractual requirements. You'll work with diverse populations, including children, adults, families, immigrants, refugees, and individuals with mental health, HIV/AIDS, or substance abuse issues.

Duties & Responsibilities:

Clinical Support:

- Provides short-term case management services.

- Receives case assignments, establish plans, and review progress.

- Conducts regular meetings with assigned cases and community representatives.

- Offers individual counseling for motivational and psychosocial issues.

Outreach and Assistance:

- Conducts outreach and intake as needed.

- Provides counseling and assistance for self-sufficiency.

- Develops, implements, and monitors service plans.

Financial Assistance and Reporting:

- Screens for income supports and advocate for benefits.

- Provides financial assistance and support per organizational policies.

- Completes forms and reports accurately and on time.

Professional Development:

- Develops relationships with social service, health, and governmental agencies.

- Participates in meetings, training sessions, and conferences.

- Contributes to program and operational planning.

Qualifications:

- BA/BSW with three years of experience or Master’s degree with one year of experience.

- LPC/LSW license may be required.

- Valid driver's license may be needed.

Knowledge/Skills/Abilities (K/S/A):

- Fluency in a second language may be required.

- Ability to read, write reports, and speak effectively before groups.

Physical Demands:

- Stand, walk, sit, use hands, and reach frequently.

- Occasionally lift and/or move up to 25 pounds.

- Specific vision abilities required.
