Job Description Summary

The Palliative Care Coordinator is responsible for coordinating the activities of the palliative care department, including scheduling and confirming patient visits, providing patients with program information, performing prior authorization checks, and other duties required to ensure patients receive the highest level of care possible. The Palliative Care Coordinator reports to the Palliative Care Supervisor and works as part of the whole palliative care team.

Essential Job Responsibilities

  1. Coordinate and facilitate the delivery of palliative care to pediatric and adult patients.
  2. Assist in scheduling appointments between patients and palliative care providers and ensures that scheduling supports the highest level of excellence in patient care.
  3. Responsible for inputting patient data into the electronic medical record with attention to detail and a high level of accuracy.
  4. Responsible for working with insurance providers and obtaining prior authorizations as needed.
  5. Acts as a liaison between palliative care patients and palliative care providers.
  6. Responsible for providing information in full to palliative care providers regarding patient requests and needs and acts promptly when directed by the palliative care supervisor to fulfill such needs.
  7. Responsible for maintaining records provided to the palliative care program from outside providers and organizations.
  8. Occasionally visits patient homes to assist in facilitating telehealth visits with palliative care providers.

Position Qualifications

  1. A Certified Medical Assistant licensure in the State of Washington is required for this position.
  2. Minimum of 3 years experience in health care, preferably in a clinic setting.
  3. Experience in hospice, palliative, or end-of-life care preferred but not required.
  4. Demonstrates good verbal and written communication, and organization skills.
  5. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Benefits
  • Full-time employees accrue up to 168 hours of PTO annually, in addition to accruing sick time at the rate of 1 hour for every 40 hours worked. Bereavement and jury duty leave are also available.
  • Option to enroll in medical, dental, vision, and life insurance. Heartlinks pays for 100% of the employee's medical premiums, and 75% of the employee's dental and vision premium. Health benefits begin the first of the month after start date.
  • Miles driven for work are reimbursed at 62.5 cents per mile.
  • Holiday pay for 6 observed holidays.
  • Ability to enroll in 401(k) and begin contributing at time of hire. Heartlinks will match dollar for dollar up to 5% of earnings after one year of employment and 1080 hours worked.
  • Tuition reimbursement/professional development assistance.
  • Hourly wage dependent on experience.
