JOB SUMMARY:
Performs a variety of administrative functions. Supervises the Case Managers, maintenance of consumer files and the data base. Ensures consumers files are in compliance with State and Federal Regulations. Relies on extensive experience and judgment to plan and accomplish tasks, evaluate performance and assign work. Setting goals, objectives and follow up.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Supervise, Assist and mentor all Case Managers
- Report changes and problems to the Program Coordinator and Executive Director
- Quickly relay and follow up with Case Managers regarding staffing needs and Changes
- Assure that Case Managers are properly filing and auditing consumer files
- Assure that Case Managers are following up with Care Coordinators and other supporting agencies to obtain any proper documents that are needed.
- Request Consumer records and documentation as needed
- Review and ensure all ISP Programs are updated with renewals timely
- Ensure CMs are addressing Provider Corrections timely
- Participate in consumers Fair Hearings/Assessments as needed
- Ensure Case Managers are completing POC Meetings timely and obtaining proof of submission
- Assist CMs with writing Goals and Objectives, Amendments, Progress Reports, entering Harmony Data as needed
- Ensure Process incoming and outgoing transfers are being completed
- Review and provide oversight for all weekly/ Daily reports
- Confirm all Case Manager Visits are completed for each month and scheduled in Therap
- Facilitate Consumer meetings ensuring that all necessary program information, training and agency policy/procedures as reviewed as needed.
- Ensure program compliance, safety procedures contracts, and State and Federal regulations.
- Handle confidential and sensitive information with poise, tact, and diplomacy
- Confirm all Case Manager Mandatory Reporting is completed weekly
- Review and check Therap and Harmony for updated information, SAs, EVV, and notices as needed
- Organize and prioritize large volume of information and assignments, including paper and electronic filing through network server
- Work independently and within a team on special and ongoing projects when necessary
- Projects a positive image of organization and conduct one’s self professionally at all times, when dealing with internal and external colleagues, consistent with the values and culture of the organization.
- Perform all other duties as assigned
- Coordinates with Program Director, Executive Director, CFO and/or CEO to assure quality care for individual receiving services.
- Provide updates/follow ups on caseloads weekly or as needed to Program Director, Executive Director, CFO, CEO.
- Assist Office Manager, Executive Director, Program Coordinator, CFO and CEO as needed
KNOWLEDGE, SKILLS AND ABILITIES;
- Positive leadership techniques and tactics
- Strong interpersonal skills and customer service
- Must have knowledge of the HIPAA laws of 1996 to maintain and safeguard confidential medical and personal records
- Knowledge of various Microsoft software applications, such as MS Word, Ms Excel. Must type 40 wpm
- Multi-line phone, copiers and fax machines
- Effective written and oral communication skills
- Ability to consistently be at work and on time
- Ability to maintain a confidential working relationship
- Excellent personal organizational skills
- Ability to follow oral and written instructions
- Ability to communicate effectively and maintain a positive attitude
QUALIFICATIONS:
- Must have a high school diploma or GED with 4 years of experience in Case Management or 4 year BA/BS in Health or Social Services
- Must be at least eighteen years of age and have a valid Alaska Drivers License
- Must be able to pass a Criminal Background Check
- Must have a high level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrative poise, tack, and diplomacy
- Must be able to interact and communicate with individuals at all levels of the organization.
- Must have knowledge of a variety of computer software applications, word processing, spreadsheets, database and presentation software (Microsoft Outlook, Word, PowerPoint and Excel)
- Requires continual attention to detail and composing, typing, and proofing materials, establishing priorities and meet deadlines. Must be able to work in a fast paced environment with demonstrated ability to juggle multiple competing tasks and demand
WORK ENVIRONMENT:
- Ability to work in an office setting
- Position may require extended hours including evenings and weekends, travel to clients’ residency, and travel between office locations.
