1. Coordinates and manages the Physical Therapy Services for HEARTSHARE WELLNESS ARTICLE 16 CLINIC.
2. Evaluates patients and administers physical therapy treatments in accordance with a written and signed referral/doctor's order from a physician. Determines the patient’s habilitative needs.
3. Writes comprehensive, legible and concise physical therapy evaluations, progress notes, treatment plansand discharge summaries in accordance with facility policies, regulatory agency standards, and Third-Party insurance payers.
4. Develops and implements appropriate physical therapy treatments.
5. Involves the patient and/or family in planning objectives and goals for the treatment plan.
7. End of service/Discharge services when maximum benefit is received and formulates discontinuation and follow-up plans.
8. Demonstrates ability to appropriately utilize equipment and supervises the use of equipment, including modalities and therapeutic exercise machinery.
9. Reports the patient’s status to the physician,Residential/Day Program nurse, Sr. Diretor of Nursing at Heartshare Human Services through formal discussions and by the use of progress notes.
10. Completes daily treatment statistics forms and submits daily treatment voucher and/or checklist for all assigned patients in a timely manner.
11. Assist with trainings, in the residential and day programs, as requested by the agency.
12. Attends and participates in professional meetings, seminars and workshops in order to enhance and maintain current professional knowledge in the areas of medical/legal aspects, new treatment procedures and equipment.
13. Maintains and properly cares for physical therapy equipment and supplies.
14. Assists with inventories and makes suggestions as to equipment and supplies needed.
15. Follows established safety procedures and maintains good housekeeping habits.
16. Assumes additional responsibilities as delegated by the Medical Director.
17. Remains sensitive and ensure staff responsiveness to cultural differences in Heartshare Wellness service population.
18. Undertake any task and initiative as assigned by the Clinic Director.

QUALIFICATIONS:
Completion of professional degree program, Masters preferred.
2 years’ experience as a Physical Therapist with some supervisory responsibilities.
Knowledge of OPWDD (Office for People with Developmental Disabilities) rules and regulations preferred.
