Intake Coordinator
Overview:
In this role, you will have the opportunity to work up to 40 hours a week providing direct support to justice-involved individuals. Our Intake Coordinator provides understanding to any inquirer about Help Me Help You Foundation’s (HMHY) programming. The Intake Coordinator plays both the role of an office receptionist and the first point of contact for an incoming participant when enrolling in HMHY’s Navigation Continuum of Care. The Intake Coordinator must have the ability to relay pertinent information to the individual about the services that are offered by HMHY.
Duties:
- Responds to referrals/inquires, via email, telephone and other methods, from individuals seeking help navigating services
− Conducts initial prescreening of individual’s needs to determine what program options are suitable
− Must be efficient in Excel, Word and PowerPoint
− Demonstrated ability to obtain, document and record all pertinent information in the prescribed manner
− Ability to relay Help Me Help You Foundation’s Mission, Values and communicate necessary information about the services and program compliance
− Responsible for scheduling assessments, in collaboration with the corresponding staff member
− Must provide exceptional customer service and professional communications to participants, families, colleagues, and professional staff
− Accurate and timely completion of follow up tasks associated to professional referrals, website requests, emails, and eClinicalWorks referrals
− Ability to perform a variety of duties and work within a structured environment other duty as assigned
Qualifications:
- Associate degree or two years in college in the healthcare field, human development, counseling, public health, psychology or social work or specialized training
directly related to the program and/or population served, or equivalent experience.
- Experience with data management systems and tracking.
- Excellent people skills, with experience in collaborating in a multidisciplinary, diverse and dynamic team environment.
- Personal qualities of integrity, credibility, and a commitment to HMHY mission.
Job Type: Part-time
Pay: $15.00 - $16.00 per hour
Expected hours: No less than 30 per week
