Lead Case ManagerReports to: Chief Operations OfficerWork Schedule: Full-time, in-person. Core hours of Monday-Friday. Must be able to work a flexible schedule with the possibility of some occasional early morning, evening, and weekend work. Travel to different facilities at times will be required and working on Teams.
Position SummaryThe Lead Case Manager is an integral part of our case management team and will bring experience leading case management efforts and interacting with participants on their Individual Reentry Plans. As the Lead Case Manager, you will be the senior case manager reporting directly to the Chief Operations Officer. You will work together to ensure that policy and procedures are met in the delivery of case management services to our participants. Key AccountabilitiesProvides daily oversight to the case management team to include, but not limited to, guidance related to service delivery and documentation, maintaining compliance with regulatory standards, appropriately managing caseloads, and coordinating on-call scheduling
  • Effectively self-manages and collaborates with all levels of staff and clients with varying needs ensuring timely and efficient utilization of resources; preventing a return to homelessness and ensuring adherence to the organization and regulatory requirements; ensuring licensure compliance as well as business process improvement measures for quality assurance
  • Provides case management expertise to include but not limited to assessments, treatment planning, linking to community resources, and monitoring progress
  • Lead bi-weekly meetings, supervision meetings, and other required meetings and/or training
  • Identifies individual client services needs through ongoing outreach, engagement, screening, and assessment activities
  • Maintains sufficient contact with clients served to ensure engagement with client services; identifies emerging needs and promotes goal attainment
  • Collaborates with other departments and outside partners effectively ensuring team-oriented, client-centered focus as well as creating a safe and secure environment for clients, staff, and visitors; provides on-call coverage as necessary; advises supervisor and others as needed ensuring adherence to best practice
  • Responsible for training new hires on case management throughout the organization
Case management –
  • Assess client-specific needs, and obtain knowledge of each client’s history through interviewing records review, and consultation
  • Schedule and conduct monthly meetings with assigned participants to update Individual Reentry Plans with new goals and tasks, as well as assess the status of prior tasks to determine achievement and/or other strategies for completion
  • Assist clients in navigating external resources to achieve their goals and/or maintain program eligibility, such as communicating with parole or probation officers as applicable, scheduling with the DMV, applying for SNAP, referring participants to affordable health care and/or housing resources, etc.
Other Duties –
  • Develop relationships with local property managers, landlords, housing authorities, and developers of affordable housing
  • Identify needs that aren’t currently met in the community and continually explore new partnerships with community resources that may result in the addition of new referral partners
  • Maintain current info on housing opportunities, community resources, and applicable laws
  • Provide participants with education and resources on securing/maintaining permanent housing
  • Coordinate the application and payment of rental assistance for program participants
  • Attend and/or schedule on-site Trauma Informed training
  • Utilize Helping Hands' proprietary database for case management
  • Other duties as assigned
Physical Requirements
  • Work is primarily performed indoors, in an office setting, occasionally accompanied by a moderate noise level and exposure to scents
  • On occasion, represent the organization within the community
  • Frequently required to sit/stand at a computer or in a stationary position
  • Frequently operates a computer and other office productivity equipment, such as copy machine, printer, and phone
  • Occasionally required to move about the facility for different meetings, office equipment, etc.
Education, Skills, Experience, And Training Required For The Position
  • College Education preferred
  • A minimum of one year of social services experience – an equivalent combination of education and experience may be considered
  • Excellent interpersonal, oral, and written communication skills
  • Preferred experience working with low-income populations, and/or drug and alcohol addiction
  • Basic knowledge of Microsoft programs and/or word processing and spreadsheet software
  • Reliable transportation
Preferred Qualifications
  • Lived experience with homelessness and/or addiction
