Position Description: The Community Referral Coordinator is responsible for coordinating community referrals for care management for the four Health Homes managed by HHUNY. These referrals will be assigned to over 90 Care Management Agencies (CMAs) within those four Health Homes. This position utilizes a NYS managed database to complete the referral process and initiate data migration. The referral assignment process includes screening for eligibility criteria and working in tandem with county Single Point of Access (SPOA) offices and Managed Care Organizations (MCOs) processing community referrals. The referral coordinator will process referral assignments to CMAs and transfer members upon addition of new a CMA or Health Home. Community referrals should be expedited to the CMAs. This position will also support training needs of CMAs and referral sources regarding eligibility as well as answer the referral phone line for member and provider questions re: appropriateness of referrals, status of referrals, etc. The position requires partnering with others that have different perspectives, backgrounds, and experiences. Having an understanding of the medical and behavioral healthcare delivery system is preferred. Confidentiality is a must as the referral coordinator will be working with protected health information. This position requires the employee to live in New York State, preferraby upstate. Minimum QualificationsBachelor's Degree in human service, social work, healthcare administration or similar field of study is preferred, OR equivalent education and work experience.

$24.00 - $26.50 Hourly
