Qualifications:
Bachelor's (Preferred)
Physical Therapy License (Preferred)
CPR Certification (Preferred)
Primary Responsibilities :
Provides physical therapy to home care patients and case management when indicated.
Responsible for timely and accurate communication and documentation relative to physical therapy services.
Promotes and develops physical therapy services in homecare .
Specific Job Duties - includes, but not limited to:
Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.
Performs initial start of care assessment and all related forms.
Develop, revise and, execute an appropriate care plan for restorative and maintenance patients.
Collaborate with physicians, case managers, nurses, occupational therapists, physical therapy assistants, home health aides, and office staff to ensure consistent levels of care.
Monitor patient progress and document accordingly.
Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.
