General Description Minimum Requirements:
Active Texas C.N.A. license required
Preferred one (1) year experience in nursing,
Must possess a valid Texas Driver's License and proof of liability insurance,
Must possess the ability to administer medication and properly read vital signs,
Must possess the ability to properly enter medical information and maintain medical records and reports,
Must possess the ability to prepare and maintain medical files,
Must possess the ability to instruct, counsel, and assist patients in meeting their health care needs.
Must develop individualized plans and implement and monitor case management service plans,
Must be willing to make hospital rounds in case client needs to seek further medical attention.
Must be able to identify community services and make referrals to health care services.
Must be able to assist in evaluating services provided and make recommendations for improvements.
May train others.
Must possess good verbal communication skills - English and Spanish,
Must possess good written communication skills - English,
Must be able to work hours which do not always coincide with a Monday through Friday, 8:00 a.m.- 5:00 p.m., workday - shift work,
Must possess the ability to initiate physical screen tests,
Prefer experience in working with disadvantaged or underprivileged individuals,
Psychiatric experience and Bilingual preferred, and
Best matched candidate will be selected to fill this position.
Must have not ever been convicted of or placed on court-ordered community supervision/probation for any criminal offense classified as a Class B misdemeanor or higher.
