Coordinates services for registrants with community agencies.

Counseling with family members and registrants to resolve problem situations as they arise.

Maintains a care plan schedule for all registrants including sending notification to family members or caregivers.

Completes Social Service assessment, quarterly reviews, social history and attendance at care plan meetings.

Completes transportation scheduling of registrants to and from program.

Sets up all charts for new admissions to programs.

Charting in progress notes any changes in registrant mental health or social situation.

Completion of admission and discharge records.

Answering of the program phone and marketing of the program..

Completes totaling of monthly activity stat sheets for Coordinator.

Assists with activities, lunch, snack time and toileting when necessary.

Completes Health Care Proxy, DNR or Living Wills with each registrant as needed.

Attends all required in-services.

Completes monthly reports for social services on registrant’s attendance.

Participates in teamwork, individualized care, respect and professionalism.

Demonstrates enthusiasm and general interest in work
