We are searching for a Care Navigator who will assess needs, develop mental health plans, and effectively collaborate with care providers to ensure continuity of care across the spectrum of behavior healthcare, social services, housing, education, and employment for individuals in care and their family members. The Care Navigator will assess strengths and needs and, in collaboration with the individual, develop a person-centered wellness and recovery plan. The Care Coordinator will bridge any gaps in care to support goal progress for individuals and families transitioning from inpatient care to a more independent daily life. This compassionate, caring, and supportive team member will contribute to the skills improvement, achievements, and overall lifetime potential success of individuals and their state of wellbeing.

You will get to:

• Assess individual needs and collaborate with them to develop and execute effective recovery and life management plans with a high level of customer service.
• Collaborate with teammates and community, healthcare, and social service providers to offer personalized and high-quality care for individuals.
• Communicate with various parties to ensure there is continuity of care in all aspects of transition from inpatient care to independent daily life for individuals.
• Maintain awareness of progress, barriers, and potential issues for individuals within in caseload and escalate as necessary to proactively provide solutions.
• Quickly build rapport and respond to individuals and families in a compassionate and caring manner while providing customized approaches to issues and concerns.
• Translate healthcare-related verbiage into actionable instructions and attainable goals for individuals and families in care.
• Track data, documentation, and communications with individuals while maintaining required confidentiality with agencies’ electronic health records, care management systems, client data, and CCBHC requirements.
• Write and document articulately while using correct grammar so that documentation is clear and valuable.
• Maintain compliance with all legal requirements and company policies and procedures.
• Consistently meet and follow up with individuals and various providers to maintain progress as well as compliance.

You’ll definitely need:

• Experience working in a mental health or primary care setting.
• A High School Diploma or GED.
• Have a valid Texas State Driver’s License.
• Have current automobile insurance and the ability to travel to individuals when necessary.

We’ll also want you to:

• Employ excellent verbal and written communication skills to engage and report effectively.
• Be flexible and customized approaches that best serve individuals in care.
• Maintain a well-organized schedule while allowing room for flexibility and adjustments.
• Maintain accurate and timely collaborative documentation.
• Sustain a calm demeanor when under pressure to meet deadlines and handle sometimes difficult situations.
• Provide exemplary customer service and care to individuals with their best interest at heart.
• Work autonomously to complete assignments within company and state parameters with a high level of quality.
• Complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training.
