Position Summary
The Pharmacist Medication Clinic is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. In addition, will serve as a resource to the pharmacy, nursing and medical staff within the field of anticoagulation and Hepatitis C drug management.
KEY RESPONSIBILITIES
Performed majority of the time:
· Provides collaborative drug therapy management to patients referred to the anticoagulation clinic to include:
o Initiation and dose adjustment of warfarin
o Initiation and dose adjustment of Direct Oral Anticoagulants (DOACs)INR monitoring with point of care machine Compliance monitoring
o Patient and family education
o Drug/drug interaction monitoring
o Drug/food interaction monitoring
o Follow-up instructions
o Coordination with home health services as applicable in order to assure optimum patient outcomes.
o Bridge therapy for patients who have a scheduled procedure in which continued anticoagulation coverage is ordered by the referring provider.
· Effectively communicates and documents all activities occurring in the anticoagulation clinic records to include:
o Anticoagulation Progress
o Notes Communication
o Notes/Special Instructions
o Anticoagulation Teaching Records.
· Participates in all required training and proficiency testing for the laboratories point of care testing program.
· Coordinates ongoing quality assessment and improvement activities for the anticoagulation clinic.
· Participates in the training of pharmacists, pharmacy students and residents in the area of anticoagulation.
· Serves as a resource to the pharmacy, nursing and medical staffs in the area of anticoagulation.
· Once completed with Hepatitis C training modules, will provide dosing, education, and monitoring of Hepatitis C medications.
· Acquire any needed prior authorizations for expensive Hepatitis C medications.
Performed occasionally but critical to successful performance of the job:
· Participates in the educational activities and programs on anticoagulation for the hospital’s professional staff using effective and appropriate teaching methods.

Education:
Required

Preferred
· Graduate of first year residency program
· Graduate of second year residency program in Ambulatory Care.
Experience:
Required
· N/A
Preferred
· Experience in working at an Anticoagulation Clinic.
· Minimum of two (2) years of experience in hospital or ambulatory care pharmacy.
Licenses, Certifications and/or Registrations:
Required
· Current, valid pharmacy license by the Oregon Board of Pharmacy.
Preferred
· Certified Anticoagulation Provider preferred or working toward certification.
Job Related Skills, Abilities and Behaviors:
Required
· Excellent customer service skills.
· Comprehensive clinical pharmacy knowledge base with emphasis in the field of anticoagulation and Hepatitis C.
· Knowledge of and understands coagulation and thrombogenesis.
· Knowledge of and understands management of drug-drug and drug-food interactions associated with warfarin.
· Knowledge of and understands the pharmacokinetic and pharmacodynamic properties of warfarin, DOACs, and anti-virals
· Knowledge of and understands the disease processes and medical therapies affecting anticoagulation and Hepatitis C
· Knowledge of and understands the current guidelines for managing anticoagulation, associated risks, and patient assessment.
· Presents self in a professional manner.
· Demonstrated ability to communicate effectively in verbal and written form with the health care team and with patients.
· Excellent interpersonal communication skills.
· Demonstrate ability to effectively evaluate and adjust patient specific anticoagulation therapy.
· Demonstrate ability to prioritize multiple responsibilities in an ever growing, busy clinic.
· Demonstrate ability to effectively develop an ongoing relationship with the clinic patients in order to assure optimum outcomes.
· Demonstrate ability to effectively and completely document all patient and physician communications and interactions.
· Accept responsibility for patient anticoagulation therapy outcomes.
· Demonstrate ability to communicate effectively with patients of varying social, educational and cultural backgrounds.
Preferred
· Bilingual skills a plus.
