Role:

You will be tasked with delivering nutrition care and education to PACE participants. Responsibilities include assessing their nutritional status, creating and executing personalized nutrition care plans, determining nutrient needs, suggesting appropriate nutritional prescriptions, monitoring participants' tolerance and response to the nutrition care provided, and sharing relevant nutritional information with primary care staff and other Interdisciplinary Team members.

Requirements:

  • A Bachelor's degree in Nutrition or Dietetics.

  • Certification as a Registered Dietitian (RD).

  • Outstanding communication and interpersonal abilities.

  • Exceptional organizational and problem-solving capabilities.

  • Proficient in working both independently and collaboratively within a team.
