Minimum Position Qualifications:

  1. BS or BA in social work or in a related behavioral science field such as psychology sociology, education, family studies, etc., with a minimum of one year full time experience working with individuals in a human service setting after completing the BA or BS or be a certified alcohol and drug counselor. A Master’s degree in social work or related behavioral health field may be substituted for the one year of experience. Organizational skills and basic computer skills.
  2. Must successfully complete case management training within 6 months of hire and annual continuing education requirements.
  3. Must have a current driver’s license and driver’s insurance.
  4. Demonstrates ability to work as an effective member of a multi-disciplined team.
  5. Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of Children’s Home clients and staff, including a commitment to use of the Sanctuary Model and to the philosophy of All Children, All Families framework of cultural competencies.

 Position Summary:

The Case Manager Supervisor is responsible for provision and coordination of TCM services to clients. Supervises a team of professionals (Targeted Case Managers) and will monitor adherence to regulations and the accurate and prompt completion of all reporting, billing, and other required paperwork. Supervisor will also be available to provide training on topics related to Targeted Case Management to new staff and case managers across programs.

Essential Job Functions

PROGRAM SUPERVISION: 30%

  1. Monitors ongoing quality assurance of documentation to ensure adherence to regulations and accurate and prompt completion of all reporting, billing, and other required paperwork
  2. Completes all staff performance evaluations and implements agency personnel policies and procedures as necessary
  3. Monitors and managers staff scheduling and service provisions, as necessary
  4. Responsible for facilitating team meetings and biweekly Chronic Co-Occurring Physical Health Conditions (CCPHC) meetings, to ensure fidelity
  5. Responsible for assuring staff have attended all necessary regulatory trainings (CORE, SED, SUD, SMI, CCPHC) within first 6 months of hire
  6. Assures the timely and accurate staff submission of service plans/service documentation and billing reports, as required.
  7. Assists in the interviewing and hiring of staff in conjunction with the Director of Outpatient Services
  8. Assures staff is maintaining caseload expectations and communicates any barriers to Director of Outpatient Services, as necessary
  9. Assisting in program development and growth of Targeted Case Management services, specifically as it relates to relationships with community partners (ie. School districts)

DOCUMENTATION AND ADMINISTRATIVE: 25%

  1. If contact with the client and family does not occur within the required time frames, the case manager is responsible to document the reason in the case record and will continue to make effort to contact the family via telephone and letter, all of which will be documented in the record.
  2. Development of a specific care plan with the client with periodic revisions with specify the goals and actions to address the medical, social, educational, or other services needed.
  3. Documentation must adhere to contract requirements following state regulations for case records, case notes, and contain the achievement and progress on goals.
  4. Discharge plans must be completed on all clients who have had three or more service contacts.
  5. Documentation must follow timeframes as stated in the program operations manual.
  6. Any youth who is considered ineligible for services is referred back to the referral source with recommendations as to a more appropriate course of action or service.
  7. To follow the safety guidelines and practices as identified in the program operations manual when providing services and provide a daily itinerary to their supervisor or other assigned person, noting where they will be located at all times throughout the work day.
  8. Case Manager will notify parent/legal guardian or other referral agencies of all critical incidents within 24 hours.
  9. Attend supervision by a behavioral health professional who has completed case management training at least twice per month with at least one of these supervisory contacts being on an individual basis and face-to-face.
  10. Ensure client’s needs are fulfilled and follow administrative protocols for request such as vouchers, use of credit cards, training request, etc.
  11. Attend case consultation and clinical meetings.

DIRECT SERVICES: 40%

  1. Responsible for maintaining a caseload of 18-20 clients
  2. Complete additional releases of information forms with the client, family, and/or legal guardian, in order to have the ability to gather information needed in developing the care plan.
  3. A comprehensive assessment and periodic reassessment must be conducted according to the program operations manual to determine the client’s need for medical, social, educational, or other services.
  4. Assist the client and families with organizing, documenting and scheduling appointments in areas identified in the treatment plan.
  5. Provide referral or related activities to help client obtain needed services with medical providers, social providers, educational providers, or other programs and services.
  6. Coordinate home visits, discharge planning, and provide monitoring or follow up activities necessary to ensure the client’s care plan is implemented and needs are being met .
  7. To have contacts with non-recipients who are directly related that have useful input regarding the client’s past, or current functioning, symptoms, adherence to treatment, or other information relevant to the client’s behavioral health condition and needs for services.
  8. Assist with the medical needs of the client, securing guardian approval if needed for medication changes and additions, including phone calls and follow up letters as needed.
  9. Ensure enrollment of client in an educational program, attend school meetings and Parent/Teacher conferences and complete documentation accordingly.
  10. Ensure client has crisis numbers for case manager, mental health center, psychiatrist or other support that may be needed and information to call 911.
  11. Attends meetings and court hearings regarding client’s assigned to caseload.
  12. Provide transportation for clients to various appointments, court, meetings and home visits, as needed.

TRAINING/KNOWLEDGE/SKILLS: 5%

  1. Provide training to new staff and across programs, as necessary
  2. Knowledgeable of effective coping skills, crisis intervention strategies, motivational interviewing, stages of change, and community resources.
  3. Must have the knowledge and awareness of suicide prevention and have the ability to screen for suicide risk.
  4. Obtain case management training within 6 months of hire and maintain continuing education requirements pursuant to 908 KAR 2:260
  5. To maintain professional boundaries with client, family members, legal guardians, community resources, referral sources, and community partners at all time.
  6. Attend required trainings as assigned.
  7. Responsibility to be aware of, and follow, state law and respective professional codes of ethics to notify the individual identified and law enforcement officials when a client has made actual threat of physical violence against an identified victim, or an actual threat of some specific violent act, or made a statement of intent to harm themselves.
  8. Responsibility to be aware of, and follow, state reporting laws for dependency, neglect, abuse, or human trafficking.
  9. Must be able to perform the essential functions of this position with or without reasonable accommodation.
  10. Other duties as assigned by their supervisor.

Physical demands and work environment:

  1. While performing the duties of this job, the employee is regularly required to stand, walk and talk or hear; use hands to finger, handle or feel objects; and reach with hands and arms. The employee is occasionally required to sit, kneel, squat, and hold objects or carry.
  2. The employee must occasionally lift and/or move up to 10 pounds, and sometimes move and/or lift up to 100 pounds in order to utilize Safe Crisis Management techniques.
  3. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.
  4. The noise level in the work environment is usually moderate.
  5. The environment consists of indoor and outdoor activities with different temperatures.
  6. Cleaning and using cleaning materials, including chemicals are a demand of this position.
