Holy Name Medical Center is Hiring a Wound Care RN (Per Diem) Near Teaneck, NJ

A Brief OverviewThe Wound Care Nurse will function in the role of a resource to staff to facilitate and provide care to patients pressure injuries, ostomies, vascular ulcers, and other wounds and will participate in staff education and performance improvement initiatives related to wound and ostomy care.
What You Will Do
  • 1. Ensure care is delivered in accordance with the ANA Scope and Standards of Care and Practice, the Patient Bill of Rights and the ANA Code of Ethics for Nurses
  • 2. Ensure that patient care is delivered in accordance with standards established by the WOCNCB Society and evidence-based practice resources.
  • 3. Prioritize wound care consultations in collaboration with team members. Document wound care assessment data, develop/revise individualized plan of care, referrals and
  • 4. Conduct daily rounds and participate in unit-based activities including but not limited to huddles, staff meetings and relevant committees
  • 5. Participate in regular NDNQI prevalence activities. Assist the team in organizing and compiling post-survey data
  • 6. Implement and evaluate clinical and educational interventions to enhance the professional development of staff through collaboration with the Quality Department and Professional Practice.
  • 7. Collaborate with an inter-professional team, including but limited to; physicians, nurses, nutrition, physical therapy and select wound care team members, to ensure smooth day-to-day operations within respective team, ensure high quality care, proper hand-off communication and effective and efficient resource utilization.
  • 8. Routinely utilize evidence-based practice resources to support staff in patient care
  • 9. Collect, aggregate and analyze data to identify opportunities for improvement. Prepare monthly/quarterly reports, per organizational needs, as requested by stakeholders and leadership
  • 10. Participate in performance improvement activities related to ostomy and wound care to enhance patient outcomes, as assigned by wound care team members
Education Qualifications
  • Master's Degree Master's degree preferred Preferred
  • Bachelor's Degree from an accredited School of Nursing Required
Experience Qualifications
  • 1-3 years Experience in Wound Care Preferred
Knowledge, Skills, And Abilities
  • Expert clinical skills (High proficiency)

  • Interpersonal skills with the ability to establish develop and maintain cross-functional business relationships (High proficiency)
  • Ability to communicate effectively and problem solve utilizing critical thinking. (High proficiency)

Licenses and Certifications
  • CWON - Certified Wound Ostomy Nurse or currently pursuing within 1 Year Preferred
  • CWCN - Certified Wound Care Nurse or currently pursuing within 1 Year Preferred
  • NJ RN License Upon Hire Required
