Homage Hospice is looking for a Registered Nurse (RN) with strong clinical knowledge to join our amazing team. If you are an RN looking to advance in your career, this is the position for you!
Our RN Care Manager will be expected to work closely with our interdisciplinary team of Aides, Social Workers, and Chaplains, shaping a world of care for patients and families. Collaborating closely with your Director of Nurses, Program Director and Hospice Physician, you'll orchestrate excellence in the field, foster growth in the quality of care, and guarantee adherence to Federal, State, and Homage guidelines.

Duties:

- Provide leadership and guidance to clinical staff
- Conduct regular performance evaluations and provide feedback
- Develop and implement policies and procedures to improve patient care
- Collaborate with healthcare providers to ensure effective patient treatment plans
- Monitor clinical activities to ensure adherence to protocols and standards
- Assist in training new clinical staff members
- Manage scheduling and staffing assignments

Qualifications:

- Assoc/Bachelor's degree in Nursing or related field required
- Active RN license in the state of practice
- Minimum of 3 years of clinical experience in a healthcare setting is preferred
- Strong knowledge of case management
- Proficiency in cardiac catheterization, gastric lavage, IV insertion, patient assessment, and administering injections
- Excellent communication and leadership skills

Pay: $90,000.00 per year
