We are looking for a licensed Physical therapist for part time full time or PRN to visit patients in their home and provide home exercise program, safety evaluation, and promote and prevent falls and improve strengthening, ROM and muscle education. 

We receive referrals in the following areas: Oak Park , Forest park, Elmwood park
