Our goal is to be our patients' primary care provider through the rest of their life, by creating long-standing relationships with our patients in order to help them live as healthily as possible. With Twin Cities Physicians, you'll provide a personalized touch in guiding your patients' care. plan, bringing our services to all types of senior living communities.
Rounding Geriatric Primary Care Physician Overview:

• Provide on-site primary care services, completing 12-18 daily visits, to patients in senior living settings
• Establish and maintain trusting relationships with your patients across their care continuum
• Collaborate with APPs to build care plans, while offering recommendations and teachings for more effective care
• Maintain patient care in place; reducing polypharmacy and avoiding patient hospitalizations

Rounding Geriatric Primary Care Physician Profile:

• Unrestricted Physician license through the MN Board of Medical Practice is required
• Passion for, and knowledge of, the Geriatric population best care practices
• Prior experience rounding in Senior Living environments, or as a Hospitalist, would be beneficial
• Strong communication skills are necessary for communicating with patients and community health staff
• Valid Driver’s License and current Auto Insurance are necessary
