to be responsible for our home health clients and nursing staff in the Central/Eastern region (Kennebec and Somerset Counties). We specialize in 1:1 nursing care for individuals with medically fragile health conditions.
This unique and pivotal nurse manager position utilizes a case management model of nursing leadership by case managing a client caseload as well as the team providing care for those clients. This position is responsible for day-to-day clinical operations and management for the designated geographic region. We are looking to fill this position full time.
Specific job functions include but aren’t limited to:
· Manage a caseload of clients in the assigned region. Advocate for client and family needs
· Complete and manage documentation within specified time frames such as authorization requests and plans of care
· Participate in process improvement plan to assure that regulatory and agency policies are followed, while modeling organizational values of Compassion, Excellence, Relationships, Integrity and healing
· Make decisions that add value to HHH while achieving quality and fiscal responsibility
· Establish and implement care plans for new clients

Education & Experience Requirements: Valid Maine RN license; solid clinical background (ICU, ER and/ or ventilator experience a plus); previous home health, case management and leadership experience is preferred. Bachelor of Science in Nursing (BSN), or an Associate Degree in Nursing with a Bachelor’s Degree a related field.

Superior clinical assessment and supervisory skills required, combined with a business focus dedicated to quality, customer service, and efficiency.
Must possess strong communication skills (empathetic, yet decisive and firm when needed); excellent boundaries; strong organizational skills; ability to work independently; excellent problem-solving skills with the ability to recognize potential issues before they escalate.
Proficient in MSOffice and use of secure networks.
Must have a valid Maine Driver's license, reliable transportation, auto liability insurance and a good driving record. Must pass a criminal background check.
Location
The RNM works primarily out of their home office and travels throughout the assigned region. Requires travel to client homes and visits to the main office in Smithfield as needed.
