Home Instead, Salem, OR is Hiring a Client Care Coordinator Near Salem, OR

Home Instead is an agency that provides in-home care to seniors in Marion and Polk counties. We are seeking a qualified individual to join our Client Care Team as a Full-Time Client Care Coordinator. This position includes a significant amount of time in the field meeting with clients and assessing their care needs.

Overview :

  • This position is expected to perform a variety of duties in client services.
  • This position requires the exercise of discretion and independent judgment regarding the significant matters of developing relationships with our clients and their representatives and of maintaining ongoing billable services for our senior clients.

Objectives:

  • Meet with prospective clients to discover their needs, sign them up for services, and establish a relationship.
  • Ensure satisfaction, safety, and quality care through regular phone contact with clients and representatives, as well as in-person visits in their homes.
  • Uncover opportunities to protect and expand service hours with the aim of improving the overall value we provide to clients.

Reporting Relationship:

  • Report to the Office Manager daily. Use the Office Manager as a first point of contact for most questions pertaining to client responsibilities. Other staff are always available as well.
  • Provide daily updates to the Office Manager, via electronic means or live conversations, regarding important client issues. Initiate ongoing and constant contact.

Primary Responsibilities:

  • Communicate clearly and concisely the purpose of in-home care, and be able to easily convey the services and scope of this agency
  • Conduct Care Consultations, assess a person's needs, and sign up with services
  • Develop and maintain positive relationships with all clients through monitoring visits, phone calls and cards
  • Conduct client monitoring visits in person or by phone on regular pre-determined intervals, according to the Oregon Administrative Rules (OARs), or more frequently based on their level or type of care, as deemed necessary
  • Keep clients' families and/or representatives updated by phone, email, or mail
  • Address client concerns
  • Demonstrate open and effective communication with owners, colleagues, and Care Professionals
  • Perform on-call duties after normal business hours, when needed
  • Assist as a backup for weekend care consultations in urgent cases. These care consultations will be scheduled at the discretion of the Office Manager.
  • Other duties as assigned

Job Type: Full-time

Pay: From $25.00 per hour
