  • You have at least 1 year of Experience with Coordinating, Supervising, or related in Home Care, Home Health, Health Care?
  • You have a bachelor’s degree?
  • You have excellent attention to detail, computer skills, and people skills?
  • You have a valid D.L., vehicle with insurance, & clean driving record?
  • You can pass a criminal background check?

Your Schedule!

  • (8:30am – 5pm) Monday - Friday

Your Responsibilities!

  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
  • Conduct client/CAREGiver introductions with every new client and with every new CAREGiver.
  • Create and maintain client and responsible party records documenting all quality assurance meetings.
  • Plan and execute a schedule for quality assurance home visits.
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • Monitor, mediate, and log all client and CAREGiver activity utilizing the software system.
  • Participate in a fast-paced, team-oriented environment.
  • Demonstrate open and effective communication
  • Maintain regular attendance at the office to execute job responsibilities
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
  • Assist as a trainer to CAREGivers in various topics while offering support and coaching.
  • Participate as needed in quarterly CAREGiver meetings
  • Assist with after hours on-call scheduling coordination periodically.
  • Reflect the Company values adhere to all policies and procedures.
