Responsibilities
- Deliver exceptional nursing care to patients in their homes, following established care plans and protocols.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patients' vital signs and health status.
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
- Educate patients and their families on disease management, self-care techniques, and treatment options.
- Assist with activities of daily living, including bathing, grooming, and dressing.
- Perform wound care and dressings as required.
- Maintain accurate and up-to-date patient records.
- Valid and active nursing license in the state of Illinois.
- Knowledge of medical terminology, medications, and common diseases.
- Excellent communication and interpersonal skills.
- Ability to work independently and make sound nursing judgments.
- Strong organizational and time management abilities.
