Hope Health Care is Hiring a Care Advocate (FT) Near Garland, TX
- Visiting clients, patients, and patient families in their homes to initiate services in the DFW area.
- Advocating for clients’ patients upon hospitalization.
- Determining other potential services patients may be eligible for.
- Coordinating with physicians, case managers, and caregivers, assisting patients to access the right care and service at the right time.
- Demonstrate excellent interpersonal skills and be an excellent communicator.
- Possess the desire and skill set necessary to work with patients with complex needs up to and including end of life.
- Demonstrate Hope Health Care’s agency values – compassion, dignity, integrity, professionalism, balance, and growth.
- Education: Bachelor’s or Master's degree in social work.
- Licensure: Must have a valid Driver’s License. Must have a valid Social Work License in good standing.
- Experience: 2 years of patient care experience, community-based needs, and resourcing.
- Skills: Medical Terminology, excellent customer service skills with effective verbal and written communication, organized, flexible, adheres to HIPPA regulations. Highly skilled in attention to detail.
- Transportation: Must have reliable transportation with the ability to travel to various patient care settings. Must have valid liability auto insurance.
- Criminal History: Must agree to a drug test and pass a criminal history check.
