Hope Health Systems, Inc. is Hiring a School Based Prevention and Support Service Specialist Near Baltimore, MD

Duties and Responsibilities:

  • Specialist will be working one-on-one with clients and families to provide case management, prevention services/groups and care coordination at Baltimore City/County Schools
  • Coordinating prevention programs in 3-4 schools
  • Advocating for families,
  • Attend all staff meetings and trainings ,
  • Conducting home visits,
  • Creating, updating and maintaining resource binders,
  • Assist clients in completing job, housing, benefits, DORS, etc applications,
  • Educate team about current resources,
  • Review internal referrals with school based clinicians with specific client needs identified (ID and birth certificate or housing resources, etc.),
  • Track City Wide data for city clients for Behavioral Health Systems Baltimore,
  • Aid in transportation to doctors appointments as identified at weekly MTS meetings,
  • Working alongside treating psychiatrist/therapist and interdisciplinary team to develop goals and objectives,
  • Required to write progress notes that summaries each contact with every client,
  • Additional contact through telephone or conversation with the treating therapist may be required, In these cases, documentation on a separate contact note is necessary as a collateral contact or support linkage if the client is present,
  • Scheduling and rescheduling appointments,
  • Complete Intakes, Initial Evaluations and initial sessions with families
  • CMs will work on rehabilitation activities such as:
  • Age appropriate self-care skills including personal hygiene, grooming, nutrition, dietary planning, food preparation and self-administration of medication.
  • Social skills including community integration activities, developing natural supports, developing linkages with, and supporting the client's participation in community activities.
  • Independent living skills including maintenance of the client's living environment, community awareness, mobility skills, money management, and accessing available entitlements.
  • Activities that support the client's cultural interests, conflict resolution, anger management, interactive skills with peers and authority figures, maintaining personal living space, maintaining age-appropriate boundaries, maintaining personal safety in a social environment and time management including constructive use of structured and unstructured time.

Job Requirements:
1) Education at a minimum: Master’s degree in Social Work or other mental health discipline. Licensure as required by discipline or proof of working towards licensure within two years.
2) Work Experience: One year of experience in the Mental Health Field providing individual, group or family therapy. Internships are accepted as experience.

3) Full background check: Potential staff members must pass a full state and federal criminal background check.
4) Valid CPR and First Aid certifications
5) Must have valid driver’s license, personal vehicle and valid automobile insurance at all times. This is a mobile position.

Position includes Mental Health Therapists with the following licensures : LASW, LGSW, LGPC, LGMFT, Psychology Associates, NCC (or passed the NCE)
