Hope Services Hawaii is Hiring a Full-time CIS Housing Navigator (Island-wide) Near Hilo, HI

JOB DESCRIPTION: CIS Housing Navigator

Status: Exempt Reports To: Team Leader

CIS Housing Navigator will provide pre and post tenancy services for homeless clients. Responsibilities include, but are not limited to, CIS Referral form, Member consent, assessment/re-assessment too and CIS health action plan addendum providing education and support for the client and others in the client’s significant support network, linking clients to community resources to further independence and stability and working collaboratively with fellow team members. The CIS Housing Navigator will also provide quality case management to include supportive counseling, outreach, and provision of safe, decent and affordable housing opportunities, skills teaching/education, family support, service coordination/advocacy, and crisis support for CIS consumers. Ensures that client input is included in every step of the entire treatment and recovery process with a commitment to work with clients/ target population in community based settings. Assists individuals to obtain the support, skills, and services necessary to improve their level of functioning within their communities and thereby reduce hospitalization and/or recidivism to the shelter.

Essential Functions

  • Meet consumer outcomes by determining the objectives needed to reach the client’s recovery goals and identifying service needs that may or may not include contact with individuals, families, guardians, and/or significant others if the client desire
  • Ensure that eligible clients are registered with the appropriate programs and are authorized to receive services
  • Develop and ensure that each client has a current individual service plan; develop a transition/discharge plan for each client no longer meeting the criteria for the level of care.
  • Advocate on behalf of each client; coordinate, monitor and evaluate services with collateral contacts.
  • Provide supportive counseling or problem-solving activities to assist individuals in developing and maintaining the supports necessary to manage their illness
  • Provide face-to-face direct service contact for each client a minimum of two time per month or as directed/needed
  • Ensure consumers understand their rights as recipients of mental health services, including informed consent and risks and benefits of treatment on an ongoing basis
  • Responsible for the integrity of the clinical record and consumer files including the completion and maintenance of all required clinical and administrative documentation according to HOPE Services standards
  • Respect and maintain the confidentiality of the guests/clients served.

Other Duties:

  • Networks with community agencies as appropriate
  • Utilizes HOPE Services resources in an appropriate and responsible manner
  • Attends all scheduled supervision, departmental/team staff and General Staff Meetings
  • Perform other duties and accept responsibilities as assigned.

Work Environment:

This job operates in a professional office environment. Temperatures may vary as the position moves from air conditioned (AC) to open air/non AC areas, community. Some contact to persons served including but not limited to those affected by domestic violence, child abuse, substance abuse, mental illness, ex-offenders, and the medically fragile. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand; walk; use hands to reach. Occasionally lift, bend, stoop, push, and pull up to 20 pounds.

  • Case Management process. Assessment; Planning; Implementation; Coordination
  • Must be self-motivated and work well in an unstructured environment.
  • Must have strong computer skills, including word processing and database management.
  • Valid Hawaii driver’s license required and access to an insured vehicle.

Required Education and Experience

  • High School Diplima or GED
  • Four years of direct case management or supportive housing services experience working with homeless or other disenfranchised individuals

Preferred Education and Experienc

  • Bachelor’s degree in Psychology, Human Services or related field
  • Two years case management experience in nonprofit setting serving homeless individuals
  • Experience navigating healthcare, human services and substance abuse systems of care.
  • Valid Driver’s License and clean traffic abstract.

Scheduling requirements:

Regular full time classification. General business hours Monday through Friday, including some evenings and weekends. Hours and days may vary based on the needs of the organization.

Starting Annual Pay: $45,760.00/yr-$60,760.00/yr
