Horizon Behavioral Health is Hiring a Care Coordinator - School Based Services Near Rustburg, VA
Hours: full-time (37.5 hours per week), Monday through Friday during school hours
Location: Lynchburg, VA 24501
Position SummaryThe Care Coordinator will serve in the school system and will engage with all students attending both domestically and enrolled with Horizon BH services, to meet the mental health needs within the school system. The Care Coordinator will engage, conduct initial screening, provide supportive level counseling and/or education, and refer students to a higher level of care. This position will be assigned to a public school location Monday through Friday.
Required Education And Experience
Willing and available to work modified schedules (weekends, holidays, etc.) as required; Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to clients in other locations; Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.

- Bachelor's degree in psychology, social work, counseling or human services field
- One (1) year of relevant clinical experience
- QMHP-C
