Summary of Job Responsibilities:

The social worker at Hospice Care Plus is a key member of the patient and family's interdisciplinary care team. They assess the psychosocial and resource needs of the patient and family, provide psychosocial care and support to the patient and family, and carry out the plan of care to meet needs. The social worker also collaborates and consults with bereavement staff to provide grief support to patients and families and organizes volunteer support. They coordinate and facilitate discharge planning, transitions to other care settings, and changes in the level of care.

Position Qualifications:

• BSW from an accredited school of social work
• State licensure in social work
• Clinical experience and training in the counseling and case management needs of the terminally ill
• Ability to function as a team member and to communicate effectively with individuals and groups
• Excellent interpersonal skills to establish a supportive, therapeutic alliance with the patient and family, care partners, community, and members of the interdisciplinary team
• Current Kentucky driver’s license and unlimited access to a reliable automobile with appropriate insurance
• All employees must have the ability to lift and/or move a minimum of 40 pounds using proper body mechanics and safety measures. Individual job responsibilities may require lifting more than 40 pounds on occasion.
• Employees are regularly required to see, talk, and hear; frequently required to sit, stand, walk, and use hands.

To meet the needs of our communities, we offer these programs:

• Hospice Home Care: hospice care provided in the home, nursing home, or assisted living facility
• Compassionate Care Center: inpatient hospice care at our freestanding facility
• Bereavement Outreach: grief support for families as they receive hospice care and for one year after hospice services are completed
• Palliative Care Plus: a home-based consult service for patients with a chronic, progressive illness
